To enroll your child at Smart Start Childcare, your child must be between the ages of 8 weeks - 3 years.

The following forms must be completed to reserve your child’s spot at Smart Start Childcare : 

Enrollment Application, Registration Fee $75, First Week's Tuition, Child’s Medical Information and Allergies, Child’s Birthday, Parent Contact Information/Work Information, and Proof of Immunizations (121 Shot Record)
